Karadzic has had something done to his lower lip to give him a less assertive appearance.
Seeing those poor people picked off in Sniper's Alley was sickening, along with those poor kids in the Skorpions video. And then to see that face and that coiffure, it makes the stomach rise.

Well the guy has aged a bit, and lips are among the things that thin out in men. Both sexes get bigger ears and noses, and smaller hands, and lips tend to thin along with hair in males. It might not have been anything done to his lower lip.
